Nurse Case Manager – Refugee Health Job Opportunities

Rate: $30.00 per hour

The City of Sioux Falls Health Department is accepting applications for a part-time Nurse Case Manager for the refugee patients of Falls Community Health.  This position is a maximum of 28 hours per week, M-F, based on the clinic needs.

The nurse case manager engages with patients who are challenged with complex health issues and/or may have a lack of economic, social, and interpersonal resources. The nurse case manager facilitates a plan of care along with the patient to achieve health and quality of life goals.  This position also assists with scheduling their patients’ medical appointments, follow-up to ensure attendance and offering education and guidance for navigating the healthcare system.

This position will work very closely a multi-disciplinary team, including the medical and behavioral health staff, billing specialists, health insurance navigators, care coordinators and community health workers. They will also develop a close partnership with the local refugee resettlement agency.

Duties may also include accompanying patients to medical appointments outside of FCH, care planning and outreach to re-engage people in care.

Qualifications and Skills:

The qualified applicant must hold a State of South Dakota Board of Nursing Certificate as a licensed registered nurse and be able to work autonomously. 

Well qualified applicants will also have experience working with under-served populations, be able to excel in a team environment and have experience working with refugee health.

Bi-lingual preferred.
