Rate: $25.26 - $29.95 Per Hour

The City of Sioux Falls Health Department is accepting applications for an Interim Nurse Case Manager. This is a non-civil service position with access to the City of Sioux Falls’ medical insurance program. This position is 40 hours per week, Monday through Friday.

The nurse case manager engages with patients who are challenged with complex health issues or who have a lack of economic, social, and interpersonal resources. In addition, the nurse case manager facilitates a plan of care along with the patient to achieve health and quality of life goals. This position promotes education, self-management support, and timely healthcare delivery. Nurse case management is part of, and coordinates care with, the clinic’s care teams, which are interdisciplinary teams of clinical and non-clinical staff. Duties may also include accompanying patients to medical appointments outside of FCH, care planning and networking with local and state agencies.

Qualified applicants must possess computer skills (preferably with an electronic health record), excellent communication skills, and a professional attitude. The qualified applicant must hold a State of South Dakota Board of Nursing Certificate as a licensed registered nurse. Previous case management experience with diverse populations is strongly preferable. EOE.
