School Based Clinics

Falls Community Health has recently expanded to include a clinic a clinic in Hawthorne Elementary School. The clinic is staffed by a nurse practitioner and a nurse. Children from Hawthorne can be seen at the clinic with parent/guardian consent.

The services available at our school based clinic include:

• Physicals
• Immunizations
• Medical Care for acute injury/illness or chronic health conditions
• Lab tests and x-ray
• Follow up visits
• Health Education

This program also includes a pilot project with a clinic located at Terry Redlin Elementary starting in March 2009.
