Q: How do I make, reschedule or cancel appointments?
A: You can call or stop in to schedule an appointment. To make a change in your appointment time please call 24 hours ahead or as soon as possible. Please use the link on the right to find address information and appropriate phone numbers.
Update: As more people need affordable health care, we want to be to be an "open door" and serve those in need. Patient's who do not show for their appointments take time away from other patients that need to see a doctor. Due to this, beginning June 1, 2009, the clinic will not make appointments for patients who repeatedly fail to make their appointment times. If you come to your appointments, or call the clinic when you have to cancel, this change will not affect you. However, if you fail to keep your appointments, you may only be seen on a walk-in basis. This may cause you to wait longer to see a nurse or doctor.
Q: How much does it cost to see a provider at Falls Community Health?
A: The cost depends on the patient's household and financial situation. The clinic accepts Medicaid, Medicare, private insurance and self-pay patients. Self-pay asked to bring proof of income to the first appointment. Staff will then determine the cost on a sliding fee scale based on Federal guidelines.
